The complete effectiveness against tetanus of the active immunization of persons, as produced by a series of tetanus toxoid injections, was thoroughly established by the experience of the armed forces of the United States during World War II. The sole qualification which must be attached to this state, ment, however, is of the utmost importance today to the practicing physician who must treat an acutely injured patient. In order to take advantage of this immunity, the physician must be certain that such a patient has previously been given an adequate course of toxoid injections. On the other hand, the protective value of tetanus antitoxin when adequate dosage is used has also been established by experience. The latter constitutes prophylaxis by means of passive immunization, but the method involves all the disadvantages and dangers of the administration of foreign protein.

Artificially acquired active immunity is protection produced by intentional exposure of a person to antigens in a vaccine, so as to produce an active and lasting immune response. The antigens in the vaccine stimulate the immune system to produce antibodies and memory cells which are specifically directed against the antigens in the vaccine.

There are two types of passive immunity: artificial and natural. Artificial passive immunity is achieved by infusion of serum or plasma containing high concentrations of antibody. This form of passive immunity provides immediate antibody protection against microorganisms such as hepatitis A by administering preformed antibodies.

Abstract Immunity is the state of protection against infectious disease conferred either through an immune response generated by immunization or previous infection or by other non-immunological factors. The first article of this series reviewed those host mechanisms that protect against microbial invasion. Both limited effectiveness against particular pathogens together with pathogen evasion processes mean that certain infectious diseases are still a frequent occurrence; some are occupationally related with the risk to health care workers being particularly well documented [ 1 , 2 ]. Since particular occupationally transmitted infections can be prevented by immunization, this article will look at how the different vaccine types modulate adaptive responses to provide further protection. First, however, the terms active and passive immunity will be considered.
